By signing this form, you agree that:
• You are aware all services associated with any transferred Service Number will be disconnected & there may be costs / obligations (including early termination &/or porting fees) from your current provider, for which you are responsible.
• The Monthly Access Fee is the minimum amount you pay each month.
• For Sim Card Postage, up to 5 Sim Cards can be included in one batch.
• There is an activation fee of $4.50 per service.
• Timed calls are billed in 1 second increments and rounded up to the nearest cent.
• Included calls are subject to our Fair Use Policy (FUP), and are not for telemarketing business purposes (Available on our Website)
• Calls not shown, such as calls to international numbers, premium numbers, directory assistance, are charged in addition to the Monthly Access Fee.
• Your usage cycle is the 27th of every month.
• Included SMS is only to standard Australian numbers.
• Included MMS does not include video MMS.
• *If auto data top up is on, additional data usage is charged at $0.03 per MB.
• If turned off, data will stop as soon as user reaches their included amount.
• International call rates, and roaming charges are available on our website
• International Countries included in Calling add-on: China, Hong Kong, Malaysia, Singapore, UK, Germany, India, New Zealand, South Korea, Vietnam, Greece, Ireland, Indonesia, Thailand, USA
• Eligible countries for International Roaming Day Pack: Austria, Belgium, Bulgaria, Canada, China, Croatia, Czech Republic, Denmark, Egypt (Arab Republic), Estonia, Finland, France, Germany, Greece, Guernsey (UK), Hong Kong, Hungary, Indonesia, Ireland, Isle of Man (UK), Israel, Italy, Latvia, Lithuania, Luxembourg, Macedonia (Former Yugoslav Rep), Malaysia, Netherlands, New Zealand, Norway, Philippines, Qatar, Singapore, Spain, Sri Lanka, Sweden, Switzerland, Taiwan, Thailand, UK, Vanuatu.
• Charges to premium numbers are set by the number provider.
• International call rates may be subject to change without notice.

• For Withdrawal & Early Termination Charges please refer to Our Customer Terms.
You understand that:
• There is a maximum of 200GB that can be saved in your “Bank”.
You understand that:
• There is a maximum of 200 mobile services per “Data Pool” • For services to qualify for the Data Pool, they must be invoiced on the same account.
